Wheelchair Evaluation (re-do seat)

Scheduled for March 7th at 3:00. Located at 20 Southbridge Road (route 169), about 1,000 feet from route 20.

Walk in door. Straight across the hallway there is a door. If it’s open, go in. Someone will be at the desk. If it’s not open, go right through the automatic door.

Appointment is 2 hours long. First half is with OT. Second is with Tom Ouimette, t.ouimette@nsm-seating.com. He’s from National Seating and Mobility in Chicopee. Call Tom and explain that I made the appointment and what the appointment is for.

This appointment may be rescheduled to an earlier appointment if one opens up on the 7th of…February? March?
